Getting started
Access MyMFT through the online portal, or download the app by searching “MyChart” in your app store and selecting MyMFT. iPhone and iPad users can download it from the App Store, and Android users from Google Play.
With “Single Sign On”, you can access MyMFT using your NHS App details. Get help with signing up for MyMFT if you are not sure where to start.
Video appointments
Your MFT video appointment will be held on Microsoft Teams. Teams is a secure video messaging system and you will not need to create a Teams account to join your appointment. Please test your camera and microphone before your appointment starts.
Proxy access
Proxy access lets parents, carers and family members see the healthcare information of someone they care for, with that person’s consent. To make a proxy access request for someone under 16, please contact the team that looks after their care.
